Parents by filling out this form you give permission to allow your child to participate in tryouts with the Arizona Sting Basketball Club.  You understand that basketball is a physical sport and like all physical sports injuries sometimes do occur.  You will not hold the Arizona Sting Basketball Club, its Directors, Coaches or Members as well as Boys and Girls Club liable for any injury that might occur during tryouts.  Thank you
